Submitting a Complaint

You can submit a written complaint in any of the following ways:

  • Email our Provider Relations Team at: LA_PR_Dept@mcna.net

  • Mail to: MCNA Dental, P.O. Box 29008, San Antonio, Texas 78229

  • Fax to: 1-504-648-1400

If you have exhausted MCNA's complaint process and are still not happy, you may submit a complaint directly to the Louisiana Department of Health (LDH) Medicaid Management Information System (MMIS). Submit online at www.healthy.la.gov, or call 1-255-342-6908.
